REGISTRATION FOR DAMOP/DAMP 2004 STUDENT TUTORIALS 

? To be considered for financial assistance, you must register by March 26, 2004.

Please provide the information requested below and send it via email to poul.jessen@optics.arizona.edu . Just put the information in a single line in the email message, with the message title “DAMOP student tutorials”.

Please send it in the format indicated by the example: comma-separated entries.

? Information needed (in this order):

Last Name, First Name, Institution, Advisor Name, SS Number (if requesting financial assistance), Degree you are working on, Email address, Financial Assistance Requested (yes/no), Mailing Address

 Example:  Doe, John, Hometown Univ.,  M.Y.Hero , 444-44-4444, Ph.D., doe@hometown.edu , yes, 2202 Englewood Dr., Hometown, CO 55043

Notes:

• If, for some reason, you cannot send the information by email, you may fax it to Dr. P. S. Jessen, 1-(520)-621-6778, or send it by mail to Dr. P. S. Jessen, Optical Sciences Center, University of Arizona, Tucson, AZ 85721.

• If you do not give a SS number( or Tax ID number instead) and if you are awarded financial assistance, you will have to fill out a travel expense form to be distributed at the DAMOP meeting in order to be reimbursed.  This is cumbersome.

• Please be aware that in the past, DAMOP policy has been to support only one student per advisor and priority has been given to students who were not supported last year. Priority will also be given to students who provide the information in the format requested. Only students at US institutions are eligible for financial assistance.
