OSC Graduate 2009-2010 Appointments

DEADLINE TO SUBMIT SPRING 2010 Hiring Documents is November 30th, 2009

Read all information carefully to avoid any problems.

Be sure to complete all three steps:

1. Obtain your supervisor's authorization signature AND account number on the Graduate Appointment Funding Authorization Form.

2. Complete the appropriate 4 page Graduate Notice of Appointment Form from the list below.  Any employee hired as a Teaching Assistant/Associate and from a non-English speaking country  MUST submit a English Proficiency score.  Any incomplete forms will not be processed and will be returned for completion.  

3. Return completed paperwork to The OSC Academic Programs Office, room 403.

The Registration Payroll Deduction Plan (PDP) is now a separate form! Only students with .25 FTE positions qualify for the Payroll Deduction Plan.  Please visit http://grad.arizona.edu/My_GradColl to view additional information on this benefit and to enroll online.  Be sure to PRINT the confirmation showing that you have enrolled.

Assistant = prelims not taken or not passed; Associate = prelims taken and passed

Teaching Appointments are semester only appointments

  If you are a New Hire please make sure to complete the New Hire Packet - Non Benefits Eligible along with the Form I-9, Employment Eligibility Verification Packet.
