- A completed Graduate Application for admission and the application
fee. Graduate College requirements and online application (preferred
method) is available at

Note: The College of Optical Sciences and Graduate College deadline for
international application (non US Citizens) is December 1st.
- Domestic Applicants: Complete Domicile Affidavit - only if you are
claiming AZ residency.
- International Applicants: Your TOEFL Test Score (testing
information and locations http://www.toefl.org)
The Graduate College requires 550 Toefl Score, Optical Sciences requires
paper test score of 600, cbt score of
200, and Ibt Toefl Score of 80. The University of
Arizona code is 4832. The College of Optical Sciences code is 99.
- International Applicants: Completed Financial Guarantee Form
(required from any applicant who desires visa paperwork from the
University of Arizona). If you are offered admissions and funding
Optical Sciences will submit documentation of funding to the Graduate
College Admissions Office in lieu of this form.
- The Application Fee. The Graduate College Admissions will not
process applications that do not include the application fee.
Note, if you are applying to two graduate programs, you will need to pay
two application fees.

Note:
The College of Optical Sciences and the Physics Department are completely
separate departments, and admission to one does not include admission to
the other.
The Graduate College Admissions no longer requires submission of
official transcripts with graduate applications. Applicants
should submit an official transcript to the College of Optical Sciences. All
application documents such as statement of purpose, resume, letters of
recommendation and transcripts must be submitted to the College of
Optical Sciences not to the Graduate College at the University of
Arizona.
If recommended for admissions, the College of Optical Sciences will
submit official transcripts to Graduate Admissions.
