Please note that teaching appointments are by semester only, and a separate graduate student appointment form must be completed for fall and spring semesters. Beginning Fall 2016, students are limited to 1.0 Full Time Equivalency (FTE) in teaching appointments. Students who surpass the 1.0 FTE limit must complete the online Graduate Student Teaching Appointment Petition form.  Upon completion of the online form, the student’s faculty advisor will be emailed a faculty petition. Both petitions must be filled out for consideration of additional teaching appointments.

All Spring 2020 RA Funding Authorizations Forms must be submitted by 5 p.m. on Monday, November 18, 2019 to ensure timely processing and payment. Students who have not completed required documentation will be terminated from the system, resulting in late fees for the Spring 2020 semester. Please print the appropriate funding authorization form below and complete all pages, including the export control page, and obtain all required signatures before submission. The graduate student funding authorization form updated for Spring 2020 replaces all previous graduate student appointment forms.
